I hereby sign on behalf of myself, my child(ren) and my fellow participants.

I hereby give my consent to use my photographs and/or video in print and/or electronically for advertising and promotional purposes. They may be modified. No compensation will be given for the use of images taken during classes and events.

I hereby acknowledge and understand that during classes and events, myself and my fellow participants may be exposed to the possibility of injury, loss, and damage based on our participation. However, Budding and Blooming Artists has taken precautions to reduce risk. You, nor your participants will hold Budding and Blooming Artists liable for any personal injury or property damage as a result of your involvement in classes or events.

I hereby, on behalf of myself and fellow participants, voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Budding and Blooming Artists from any or all claims, actions or losses for bodily injury, property damage, loss of services or otherwise which may arise our of participation in classes or events. I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have presently or in the future.

I HAVE READ THE ABOVE PHOTO/VIDEO RELEASE AND LIABILITY WAIVER AND BY CONFIRMING ELECTRONICALLY WITH MY INITIALS, I AGREE. IT IS MY INTENTION TO RELIEVE BUDDING AND BLOOMING ARTISTS FROM LIABILITY FOR PERSONAL INJURY, DEATH, AND PROPERTY DAMAGE.
